Proceed, then mount the PhotoShop.dmg file using the disk utility (it should be your default program for dealing with these files). Once it's mounted it should bring up the image with the Setup file in there!
It probably would have helped to have had this in the download description because if I hadn't have tried it for a laugh I might never have known!
EDIT: For those of you struggling with the patch;
1. Install Photoshop CS4 from the Photoshop.dmg and choose to do it on a trial basis for 30 days.
2. Once installed, mount the disk image with the patches in it, double click on the left one (Use this to [k] Photoshop CS4) and this will run the patch utility program.
3. Go to Applications -- Adobe Photoshop CS4 and find your Adobe PS CS4 application.
4. Drag that application file onto the 'tick box' in the patch program, wait a few seconds and it will tell you your copy has successfully been patched.
5. Run Photoshop CS4 normally!
